Person Centered Planning

Seeing People First, Rather Than Their Labeled Diagnosis

This practice allows for the individual to make their own choices and develop their abilities so they can reach their maximum potential. Furthermore, it allows the person to connect with people with community inclusion and be treated with respect.

Community Living Supports

Community Living Supports are used to increase or maintain personal self-sufficiency, facilitating an individual's achievement of his goals of community inclusion and participation, independence or productivity. The supports may be provided in the participant’s residence or in community settings.

Private Residences

In these settings we provide support around the clock in their house, apartment or condominium. In most cases up to three individuals can live together.

Respite

We also provide respite care to families that need a break.

Transportation

We provide transportation. Our caregivers might use their vehicles or we provide a vehicle to the home. Rather it is to a movie, community outing or any activity, we ensure that they get there.

Advocacy

We dedicate time and allocate numerous resources to bring awareness and advocacy to the industry and to all stakeholders. Our philosophy is that the funding gets to the person actually receiving the services and the best way to do this is to create a workforce that is paid what they are worth. Turnover will drastically decrease and new people will enter the industry, thus increasing quality care.

Adult Home Help/Other Services

We assist with activities of daily living (ADL’s) and instrumental (ADL’s).

ADL's Consist Of:
  • Bathing and showering
  • Dressing
  • Personal hygiene and grooming
  • Toilet hygiene
  • Walking
  • Transferring
Instrumental ADL's Consist Of:
  • Housework
  • Preparing meals
  • Taking medications
  • Managing money
  • Shopping
